Only when you are contacted via email or phone by a UYA Director should you complete this first requirement for all waiting list applicants.

Your child applicant will be asked to submit a video (no more than) 2 mins. of the following info:
- Full name
- What city and neighborhood you live in
- "Why do I want to learn how to fly"
